Neonatal Bioethics: The Moral Challenges of Medical Innovation

Explores the complex ethical, medical, and economic issues surrounding neonatal intensive care. It traces the evolution of neonatology through three eras: innovation and individualism (1965-1982), exposed ignorance (1982-1992), and the end of medical progress (1992-present). The text highlights significant controversies, such as the Baby Doe case, which sparked debates about infant mortality, quality of life, and the role of parental and medical authority in treatment decisions. It further examines the cost-effectiveness of Neonatal Intensive Care Units (NICUs), challenges in prognostic accuracy for premature infants, and various approaches to shared decision-making in critical situations, including the moral distinction between withholding and withdrawing life support. The overarching theme is how society and the medical field have grappled with the moral imperatives created by advanced medical technology and the continuous effort to define ethical standards in the care of critically ill newborns.
